Does the PDF look like a real quote?

Yes. The template is a document format: Letter or A4 pages with a letterhead, a page footer and signatures, so the PDF prints exactly like the quote it replaces.

How are line items entered?

One line per item — description, SKU, unit, quantity, unit price — in the personalization form. Totals, discount, tax and the amount in words are computed in the document.

Can the client respond without printing?

Yes. On the web version they press accept or request a sample; the decision lands on their card and you are notified.
